Online Application Instructions
SUBMIT YOUR ENROLLMENT APPLICATION ONLINE!
St. Mary's School is pleased to announce that we are now able to receive applications through our website. To submit your 2023-2024 application online, please follow these instructions. If you prefer, you can also mail or drop off your application to the school office.
1. Download the application and student intake form by clicking the buttons below. If the file opens in your browser, press the download button in the upper right corner on each document. If it does not open, please save the file to your hard drive. Returning families do not need to fill out the student intake form for current students.
2. IMPORTANT! Save the fillable .pdf files to your computer. Do not attempt to complete the forms before saving. All information you complete before downloading the document will be lost.
3. Adobe Acrobat Reader must be installed on your device to complete the forms.
4. Fill out your information and save the completed forms to your computer. Please fill out one student intake form for every NEW student you are enrolling.
5. Upload the completed enrollment application and student intake forms below.
